ReverseProxy doesn't remove hop-by-hop header fields
from the response in ServeHTTP, but it should.

Steps to reproduce:

1. Download the attached file hop.go
2. go run hop.go

Expected output:
Hello [world]

Actual output:
Hello [world]
Upgrade [foo]

Attachments:

  1. hop.go (888 bytes)
